Create the FormClick on Forms and Create. Choose the sample Schedule an appointment. The form will get created. It is pre-configured with an image, a title and a Date Selection question. |

Set the Schedule and LimitsEdit the Date selection question and set your own schedule. You can limit the number of places available for each time slot. |

Send the Form by Email or by SMSOnce your form is ready, you can send it to a list of people. Either by email or by SMS. Create a new message and attach the form. |

Print a QR CodeYou can also print a QR code and post it on the wall of your local. When a parents drops by, they can scan the QR code and fill in the form with their smartphone. |

Post on Your WebsiteActivity Messenger forms have unique URLs. You can link them from your webiste. You can even embed them as popups. |

Visualize in a CalendarOnce you've gathered results, click on the Analytics tab. A calendar automatically gets filled with scheduled appointements. |
